Texas Health & Safety Code §181.105
Jane Ballard Wellness is committed to transparency, client rights, and compliance with applicable Texas law. In accordance with House Bill 4224 (89th Regular Session) and Section 181.105 of the Texas Health & Safety Code, the following information is provided for consumers.
Requesting Your Health Care Records
Clients have the right to access and request copies of their health care records.
To request your records from Jane Ballard Wellness, please submit a written request via email or through the secure client portal. Requests may require identity verification and will be processed within the timeframes permitted by Texas law. Reasonable fees may apply as allowed by statute.

Filing a Consumer Complaint
Consumers may file a complaint with the Office of the Texas Attorney General – Consumer Protection Division if they believe their rights have been violated.
